You took a home pregnancy test and it came back positive. Although these tests are highly accurate, the pregnancy must be confirmed through a blood test that measures human chorionic gonadotropin (hCG) levels and through an ultrasound.

At this first ultrasound, the doctor may mention the presence or absence of the gestational sac. What exactly is the gestational sac? When can it first be detected on ultrasound? What does it mean if it is (or isn’t) present? What does it indicate if a gestational sac is visible but appears empty? Here are the answers to your questions:

The gestational sac is one of the earliest ultrasound signs of pregnancy. It is located in the uterus, surrounds the developing embryo, and contains amniotic fluid. It can be seen on a transvaginal ultrasound between 3 and 5 weeks of gestational age, typically when hCG levels range between 1,500 and 2,000 mIU/mL. (Some studies have found that this range can vary significantly, with detection possible even at hCG levels between 390 and 3,150 mIU/mL.) The visualization of the gestational sac confirms pregnancy, but it does not guarantee that the pregnancy is healthy or that it will progress normally. The next reassuring sign after identifying the gestational sac is the appearance of the yolk sac, which develops inside it. The yolk sac nourishes the embryo until this role is taken over by the placenta. It usually becomes visible on a transvaginal ultrasound between five and a half and six weeks of pregnancy. However, in some cases, even if the gestational sac is detected, the yolk sac may not be visible later on.

What does it mean if the gestational sac is not seen on ultrasound? There are several possible explanations. First, the gestational age may have been miscalculated, making it too early for the sac to be visible; in such cases, the ultrasound must be repeated later. Another possibility is an ectopic (extrauterine) pregnancy. If the gestational sac is absent and hCG levels are decreasing, these findings may indicate an early miscarriage.

When the gestational sac is visible but contains no embryo—meaning it appears empty—this indicates that the embryo failed to develop. This type of miscarriage occurs early in the first trimester and often before the woman realizes she is pregnant. Risk factors for an anembryonic pregnancy include abnormal cell division, chromosomal abnormalities in the fertilized egg, an infection, an autoimmune disease, or endocrine disorders in the mother.

Finally, if the doctor identifies an empty gestational sac during an ultrasound, this may confirm that the pregnancy is nonviable and will not result in the birth of a child, as it is not progressing normally. Depending on the size of the gestational sac, it may be too early to determine whether it is truly empty. In such cases, the doctor will recommend returning for a repeat ultrasound to provide an accurate diagnosis regarding pregnancy viability.
