Progesterone is one of the hormones I monitor carefully when evaluating the menstrual cycle, ovulation and the chances of achieving a pregnancy. In my practice, I often explain to patients that an isolated low progesterone result should not be interpreted in a rush, without the day of the cycle, menstrual history and fertility context.
When I investigate a possible hormonal cause of difficulties in conceiving, I correlate progesterone with hormonal tests in fertility, ultrasound findings, confirmed ovulation and menstrual regularity. Only this complete picture can show whether we are dealing with a normal variation, absent ovulation, an insufficient luteal phase or a situation that requires additional monitoring.
What Role Progesterone Has After Ovulation
Progesterone normally increases after ovulation. It is produced by the corpus luteum, the structure that forms in the ovary after the oocyte is released. Its main role is to prepare the endometrium for possible implantation and to support the first stages of pregnancy.
I often tell patients that progesterone should not be viewed as an isolated hormone, but as an indirect confirmation that ovulation has occurred. If ovulation does not take place, the corpus luteum does not form correctly, and the progesterone value remains low. That is why a low progesterone result may mean either that the test was done too early or too late, or that ovulation did not occur in that cycle.
If pregnancy does not occur, progesterone and estrogen levels decrease, and the endometrium is shed through menstruation. This mechanism explains why progesterone is directly related to the rhythm of the menstrual cycle, irregular bleeding and the moment when menstruation appears.
When Low Progesterone Can Become a Fertility Problem
A low progesterone result becomes relevant especially when it repeats, when it is associated with irregular cycles or when it appears in the context of difficulty achieving a pregnancy. I do not recommend conclusions based on a single test, especially if we do not know exactly when ovulation took place.
Ovulation, Luteal Phase and Corpus Luteum
The luteal phase is the period between ovulation and menstruation. During this stage, progesterone should increase enough to transform the endometrium into an environment favorable for implantation. If the luteal phase is very short or if the hormonal level is not adequate, spotting before menstruation, shorter cycles or suspicion of insufficient endometrial support may appear.
In my practice, I monitor not only the progesterone value, but also cycle regularity, signs of ovulation and the appearance of the endometrium on ultrasound. Low progesterone can be a clue, but the diagnosis is built from several elements.
Implantation and Endometrial Support
For implantation, the endometrium must be receptive within a well-synchronized interval. Progesterone contributes to this transformation. If the level is insufficient after ovulation, the endometrium may not reach the maturity needed to receive the embryo in optimal conditions.
This discussion also becomes important in assisted reproduction. In In Vitro Fertilization protocols, hormones are carefully monitored, and luteal phase support is adapted to each case. That is why, when I explain hormones in IVF and treatment success, I insist that progesterone must be interpreted in relation to the entire protocol, not separately.
Signs That May Suggest an Imbalance, but Do Not Establish the Diagnosis by Themselves
There are symptoms that may raise suspicion of a hormonal imbalance, but they are not specific only to progesterone. Irregular periods, spotting before menstruation, very short cycles, difficulty achieving a pregnancy or recurrent pregnancy loss may justify medical evaluation.
However, I do not consider it correct to automatically attribute these manifestations to low progesterone. The same symptoms can appear in polycystic ovary syndrome, thyroid disorders, hyperprolactinemia, weight changes, intense stress, perimenopause or other gynecological causes.
That is why I recommend caution in interpretation. A laboratory result must be read in its clinical context. Sometimes, the value is low because ovulation was delayed, not because there is a real luteal phase problem. Other times, the test correctly shows that ovulation did not occur, and the medical direction must be to identify the cause of anovulation.
How I Recommend Progesterone Should Be Evaluated
The timing of testing is essential. Progesterone is usually measured approximately 7 days after ovulation, not automatically on day 21 for all patients. Day 21 may be appropriate only for a regular 28-day cycle, with ovulation around day 14. If ovulation occurs later, a test done on day 21 may falsely show low progesterone.
During evaluation, I usually assess:
- the length and regularity of the menstrual cycle;
- confirmation of ovulation through ultrasound or tests correlated clinically;
- progesterone measured at the right time;
- TSH, prolactin, FSH, LH, estradiol, AMH and other tests, depending on the case;
- history of pregnancies, pregnancy losses or previous treatments.
In the case of patients who are undergoing or considering In Vitro Fertilization from the perspective of the patient experience, I explain from the beginning that progesterone can have a different role depending on the treatment stage: ovarian stimulation, endometrial preparation, embryo transfer or post-transfer support.
Treatment should not be started only because there is an isolated low progesterone value. Progesterone supplementation can be useful in certain contexts, but it must be medically recommended, with the dose, form of administration and duration adapted to the situation. Self-medication can delay the correct diagnosis and can mask other causes of infertility.
